5) Diagnostic Imaging:
In-house digital x-ray allows us to see the exact condition & position of your spine.

What to bring
- Photo ID and calander for scheduling.
- Download and complete the new patient intake form above, otherwise arrive 30 minutes prior to scheduled appointment time.
Insurance & payment
We keep billing simple. We can provide superbills for out-of-network reimbursement. HSA/FSA accepted.
